This form is for organizations that already have a website and want it rebuilt properly. Its job is to get us everything we need in one pass so the rebuild starts fast and you're not chasing details later.
Answer in plain language. You don't need to know anything technical to fill this out. Where a question does touch on the technical side, we've told you exactly what we mean and given you room to say "not sure, help me find it." That's a real answer, and it's a normal one.

The Basics
Who you are and who we talk to. Short section.
In one or two sentences, what does your organization do and who is it for?

Access & Ownership
The plain-English version of the "technical" questions. We only need to know who controls the web address and where the site lives, so we can move things over without a scramble at launch. "Not sure, help me find it" is a real and normal answer.
Your web address (the domain, like yourorg.org). Do you own it, and do you know where it was bought?Common places: GoDaddy, Google/Squarespace Domains, Namecheap, Wix. "Not sure" is fine.
Do you have the login for wherever the current site is built or hosted?WordPress, Wix, Squarespace, a hosting company, or "a volunteer set it up and I'm not sure."
Do you use email addresses tied to your domain (like you@yourorg.org)?If yes, list every address you use in the box below, and tell us which ones need to keep working through the rebuild. If you're not sure, just say so.

Is there a date the new site needs to be live by, and is it tied to anything?An event, a campaign, a launch, a fundraising push. If there's no hard date, say so.

Your Files
